Moved questions up; changed progress formulas

Moved questions: Moved the first row of requirements below the question section

Changed progress formulas: Now column F contains the percentage associated with the corresponding progress word in column C.

Now it is easier to change progress formulas, it is just about changing any of them and copying and pasting in the whole column F.

Added the status "planned" = 5% in all progress formulas
